Oak Hills Living Center offers a variety of services to meet the needs of each individual resident. A comprehensive assessment determines the level of care required for each individual. Oak Hills Living Center offers a number of services, including:

Skilled Nursing

Oak Hills Living Center provides a warm, home-like atmosphere. With 94 rooms, we are able to give each resident the individualized care they deserve. Our amenities and daily activities provide residents with familiar activities within a safe, secure environment. Licensed and certified staff members, on duty 24 hours a day, provide skilled and compassionate care.

Assisted Living Apartments

Assisted living offers services such as meals, laundry and personal grooming assistance. Each apartment is spacious and comfortable, with a private bathroom, living area and kitchenette. 24 hour staffing provides round-the-clock services as needed. 16 apartments are designated as private pay and 16 were built thru a HUD grant; therefore have age and income guidelines.

Home Health Care

Oak Hills Living Center Home Care is a new service provided to the New Ulm community and surrounding area. Home Care provides skilled nursing services, home health aide services and physical and occupational therapy in the home following surgery, hospitalization or nursing home stay. Our home care is a Class A, Medicare certified agency and can bill insurances, and Medical Assistance as well as private pay. Our home care was established to provide a continuum of care providing services that exemplify the mission of Oak Hills Living Center, and again an effort to bridge generations.

Short-term Rehab

Oak Hills has dedicated 20 private rooms for our short-term rehab residents. These individuals are typically here for a short period of time after a surgery or accident. The goal is to come in and receive intense physical therapy, occupational therapy or speech therapy, depending on the need, so they can return back to their home within 3-4 weeks. Short-term rehab is located on Deer Haven, which has a beautiful atrium with trees and a sky light to bring in the sunlight. We have 20 large private rooms with walk-in showers in 9 of those rooms.


Hospice services are provided to anyone whose life expectancy is likely to be six months. In response to the need for Hospice in the community Oak Hills has set aside rooms to provide this service.   Quality of life is the main focus of Hospice Care. Staff will work closely with the hospice team the family already has, welcoming them into their living environment or Oak Hills will assign a team. Together, the Hospice team and the staff at Oak Hills will provide medications, equipment and supplies to support the hospice diagnosis, as well as compassionate staff to attend to personal care needs. It is important for hospice patients and their loved ones to be able to stay close to family and friends during end of life so they can maximize and savor their remaining time together.

Adult Day Services

Adult Day provides a safe, secure and familiar environment for seniors with health conditions or physical limitations that are looking and needing social interaction. The Adult Day Services Program offers social, recreational and supportive services to seniors who live alone, or as a dependent relative or friend.  Clients enjoy nutritionally balanced meals and snacks, and a variety of activities including discussion groups, community outings, guest speakers, crafts, exercises, games, movies and parties.  The benefits of joining an Adult Day program include:

  • Promoting each person’s maximum level of independence
  • Support for families and caregivers, including respite and education
  • Opportunities to socialize and meet new friends
  • Participation in Brown County’s Elderly Waiver and CADI programs and contracts with Veteran’s Administration
